Important information for your upcoming appointment. Please either bring a list of all your medications or bring your medication bottles with you.
BILLING AND INSURANCE:
Dermatology is considered a Specialty Practice; please check with your insurance company to see if a referral is required.
Your co-pay will be due at the time of arrival when you check-in at the registration desk. (If your insurance card has a regular co-pay and a Specialist co-pay, the Specialist amount will be what is due.) A photo ID will also be required at check-in. We do not need a copy of your social security card but will need your social security number.
Please bring all your insurance cards with you. Our office will file for two insurances only. We do not file the 3rd insurance. If you have a 3rd insurance, we will be happy to give you an itemized statement to file to them.
Any charges not paid by your insurance company(s) after 45 days will become the patient’s responsibility.
All minors must be accompanied by a parent or legal guardian on the first visit. If the patient or guardian is not the cardholder of the insurance company, you must have the cardholder’s address, birthday, social security number, employer, etc.
We can see minors on subsequent visits without a parent or guardian as long as we have written permission from the parent/guardian to treat them and as long as you send a co-pay and insurance information with the minor.
Cosmetic procedures can be areas that we take off at the patient’s request but pose no medical threat to the patient. These can be skin tags, age spots, benign moles, etc. Typically, insurance companies do not pay for these procedures so they must be paid in full at the time of service. (Our billing office can give you an estimate of what these costs would be if requested.)
